Patch release with no migrations and no required configuration changes.

- Reconciliation stopped at the first repository whose default branch GitHub
could not read (for example a renamed branch answering 404), skipping every
later installation on every tick. The failing repository is now marked
`error_code=default_branch`, no index job is queued for it, and
reconciliation continues; the mark clears on the next successful read.
Refresh and webhook reconcile failures now log the installation, repository,
