The scope line still named two endpoints. The changelog described the Responses streaming design before review: per-part buffers, the flush scope of output_item.done, the no-terminal-frame path and the prior assistant turns are what actually shipped.

Follow-up to #70. The 'What it does not do' scope line still said two generation endpoints, and the changelog entry described the design as first written rather than what four review rounds produced: per-part hold-back buffers keyed by item_id/output_index/content_index, output_item.done flushing only its own item, every end-of-stream path flushing what is held, and prior assistant turns (output_text/refusal) redacted on the way back up.
