Docs only (one tasks.md). Found while reviewing the review-record reader (#107): the record lives in the author's working tree, so as a bare file it proves nothing, because the author's model can write its own LGTM. Phase 1b makes the identity claim true: written only by a tool with the authenticated reviewer identity, the path fenced against model writes, the driver refusing reviewer == author, independence surfaced to policy, and an observed-red forgery test.

A review record in the author's working tree proves nothing on its own; the
author can write its own LGTM. Phase 1b makes the identity claim true: written
only by a tool with the authenticated reviewer identity, path fenced against
model writes, driver checks reviewer != author, independence surfaced.
