trodden doctor says Codex is connected before Codex trusts the hooks #186

Codex runs a new hook only after the user trusts it (at startup or in /hooks). trodden connect codex prints that step, but trodden doctor and trodden status report Codex as connected as soon as the hooks are in hooks.json, even if Codex never runs them.

Applies to the agent support in #149.

Fix idea: read Codex's hook trust entries in config.toml (read only) and report Trodden's hooks as installed but not yet trusted.
