Codex: a command whose output contains "rejected by user" is recorded as interrupted #172
The Codex parser checks the whole output for "rejected by user" / "aborted by user" before reading the exit code header. rg -n 'rejected by user' src with Process exited with code 0, or a test run that prints such a string, becomes interrupted, and a real passing check can drop out of the trace.
Fix idea: use the exit code header when there is one, and only treat header-less outputs (the refusal text Codex writes instead of running the command) as interruptions.
