Cline: a passing command counts as failed when its output mentions an exit code anywhere #171

Cline reports no exit code, so the adapter searches the result text for exit code N, anywhere in it. A passing npm test whose log says child process exited with exit code 1 (retrying) is journaled as failed with an error signature, and an error recall fires.

Applies to the agent support in #149.
