Anthropic returns 400 when a tool_result is not in the user message directly after its tool_use, is preceded by other content (images/text between results), or refers to a tool_use that is not in the previous assistant message. History was only sanitized at write time (validateAndFixToolResultIds, which neither orders results nor handles later history changes), and getEffectiveApiHistory only dropped orphan results on the summary path. Retries re-sent the same invalid payload.
Fix
New pure normalizeToolResultPairing is applied to the history built for each API request (after consecutive-user merging, before image stripping). For every assistant turn with tool_use:
results are moved ahead of other content, in tool_use order, duplicates dropped;
missing results get a synthetic "interrupted" error result (an extra user turn is inserted if another assistant turn follows);
results with no matching tool_use in the previous assistant message are dropped (a user message left empty is removed).
Stored history is not mutated; a trailing assistant turn is left alone.
Root cause (write side)
pushToolResult in presentAssistantMessage appended each tool's images straight after its own result in the flat userMessageContent. With parallel tool calls that gives [result A, image A, result B], the exact shape in the report. pushToolResultToUserContent now inserts each new result after the last existing result (ahead of images/text), so new history is written correctly.
Limitations
Other causes of orphan or missing results (interrupted tools, condensing, already-corrupted stored history) are covered by the send-time normalizer above rather than a write-side change.
Tests
New normalizeToolResultPairing.spec.ts (interleaved images, ordering/duplicates, interrupted calls, assistant-after-assistant, orphans accept/reject, well-formed history unchanged, no mutation). core/task suites (929 tests) and check-types pass.
Improved the ordering and grouping of tool results in conversation history.
Kept images and other message content from interrupting tool-result sequences.
Removed duplicate and unmatched tool results, and supplied interruption results for tool calls without a matching result.
Preserved the relative order of non-tool content while keeping valid conversation history intact.
The change normalizes tool-result pairings in API conversation history. It orders matching results, removes duplicates and orphans, and adds synthetic interruption results for missing calls. Task also places new tool results before existing images or text.
Changes: Tool Result Pairing
Layer / File(s)
Summary
Normalize tool-result history src/core/task/normalizeToolResultPairing.ts, src/core/task/__tests__/normalizeToolResultPairing.spec.ts
The new helper orders matching results, removes duplicate and unmatched results, inserts synthetic interruption results for missing calls, and preserves other user content. Tests cover these cases and confirm that normalization does not mutate the input history.
Integrate normalization in Task src/core/task/Task.ts, src/core/task/__tests__/Task.spec.ts
Task inserts new tool results before existing user content and normalizes API conversation history before image removal. Tests verify that tool results precede images and text while other content retains its relative order.
If saving a delegated task’s pending tool result fails, resuming the parent can send an invalid request and stall the task. Prevent that continuation before merging.

Explanation
Only directly linked issue #2098 applies. Its coding requirements are: (a) tool_result blocks first and contiguous in the user turn after the tool_use turn, in tool_use order, before images or text; (b) synthetic error results for interrupted tool_use calls before the request is sent; (c) orphan tool_result blocks dropped on every request path; (d) retries must not resend the invalid payload. Evidence: src/core/task/normalizeToolResultPairing.ts at the reviewed head builds results in tool_use order (toolUseIds.map(...)) and places them ahead of the remaining user content ([...results, ...rest]). It adds interruptedResult (is_error: true) for each unmatched tool_use. It drops tool_result blocks in user messages that do not follow a tool_use turn, and drops tool_result blocks that match no tool_use in the previous assistant message. It keeps a trailing assistant turn unchanged. It inserts a user turn when another assistant turn follows. It builds new message objects and does not mutate input. Per the PR summary, Task.ts applies the normalizer to the merged request history before image stripping, so the fix applies to every request, and pushToolResultToUserContent inserts new results after the last existing result (ahead of queued images). Added tests in normalizeToolResultPairing.spec.ts and Task.spec.ts cover these behaviors. The test run was not independently executed here. The incremental diff from the previous reviewed head could not be read, so Task.ts and the test files rely on the supplied summary and the prior assessment. The normalizer source was read directly at the reviewed head.
Full details: Out of Scope Changes check
Explanation
All changed production code serves issue #2098. normalizeToolResultPairing.ts adds send-time history shaping for tool_use/tool_result adjacency. Task.ts applies that function to request history and changes pushToolResultToUserContent result placement for parallel tool calls. The tests exercise these behaviors. No unrelated refactor or feature was identified in the supplied summary or in the normalizer source read at the reviewed head. Uncertainty: the incremental diff from the previous reviewed head could not be read, so any changes made since that head are assessed from the supplied summary only.
Full details: Security Boundaries
Explanation
No changed path meets a security-boundary failure condition. src/core/task/normalizeToolResultPairing.ts only reorders, deduplicates, drops, or synthesizes fixed tool_result blocks for outgoing history; it does not execute input, expose secrets or PII, or alter approval and allowlist decisions. Task.pushToolResultToUserContent only changes block placement. The changed request path remains history shaping before API submission, while tool validation and approval handling remain in the existing presenter path.
Full details: Persistence Integrity
Explanation
No changed persistence path introduces the stated failure conditions. The PR changes in-memory tool-result ordering at Task.ts:585 and applies a pure, non-mutating request-history normalizer at Task.ts:5839; neither writes stored history. The existing flush path still awaits saveApiConversationHistory() at Task.ts:1582 and retains pending results when the save fails. No changed write is non-atomic, unawaited, or missing rollback/partial-failure handling.
Full details: Lifecycle Resource Cleanup
Explanation
No changed lifecycle path can leak a resource or duplicate work. The PR adds a pure, synchronous normalizeToolResultPairing transformation and calls it while building API history; it creates only in-memory arrays and synthetic blocks. pushToolResultToUserContent only changes array insertion order and duplicate-ID handling. The changed lines add no listener, watcher, provider, timer, task, subscription, or disposal/restart behavior.
Full details: Description check
Explanation
The description is relevant and mostly complete. It links issue #2098, explains the problem, implementation, root cause, limitations, and tests. The test information is sufficient, but the template checklist and optional sections such as documentation updates and contact details are not included.
Full details: Regression Evidence
Explanation
The send-time request behavior lacks focused boundary coverage. Task.ts:5834-5841 now applies normalizeToolResultPairing before image removal, but the request tests at Task.spec.ts:2923-3079 cover only ordinary fields and image shaping; they do not put malformed tool history in apiConversationHistory and inspect the history passed to createMessage. The new tests cover the pure normalizer and the write helper (normalizeToolResultPairing.spec.ts:21-85, Task.spec.ts:8203-8307), but they would pass if the request call were removed or ordered incorrectly. This is the changed behavior identified by the standing failure.
Resolution
Add a focused attemptApiRequest test. Seed apiConversationHistory with assistant tool_use blocks and interleaved, duplicate, missing, and orphan tool_result blocks. Capture the history passed to createMessage and assert contiguous tool results in tool-use order, synthetic error results, orphan removal, image handling, and unchanged stored history. Keep the existing normalizer and write-path unit tests.

The condense request path does not send raw tool blocks to the summarizer. summarizeConversation converts every tool_use and tool_result block to text before calling apiHandler.createMessage, so Anthropic tool-result adjacency validation cannot fail on that request. The comment’s premise is contradicted, and no normalization change is required for this path.

Add the required boundary regression test for string-content user turns.
A string-content user turn can follow an assistant tool_use. toBlocks converts it to a text block before inserting the synthetic result. The current test uses array content only, so it does not protect this branch. Add the focused test below. Do not add is_error until the production implementation adds that field.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win
Mark synthetic interrupted results with is_error: true.
The synthetic tool_result built here has no is_error flag, so the model sees it as a successful result with the text "Tool execution was interrupted before completion." This conflicts with the existing convention in src/core/task/Task.ts (resumeTaskFromHistory, lines 3295-3305 and 3340-3347). That code sets is_error: true on interrupted-call results so that "the persisted history cannot be misread as a successful completion of the interrupted tool calls (e.g. attempt_completion)" (issue #1283). The PR objective and issue #2098 also ask for synthetic error results.
Trigger: an assistant turn calls attempt_completion (or any tool) and the result is lost because of cancellation, compaction, or delegation. Normalization then answers it with a non-error result, and the model can treat the interrupted call as completed.
Add is_error: true. Also update the expectation at normalizeToolResultPairing.spec.ts line 37.

Anthropic rejects requests where a tool_result is not directly after its
tool_use, is preceded by other content, or has no matching tool_use. Shape
the history sent to the API so results come first in tool_use order,
interrupted calls get a synthetic error result, and orphan results are
dropped, on every path rather than only after a summary.

Context-management requests do not send raw tool blocks.
manageContext sends history to summarizeConversation, which injects missing results and converts every tool_use and tool_result block to text before calling apiHandler.createMessage. The reported pre-normalization Anthropic 400 path is prevented.
5839-5839: 🩺 Stability & Availability
The comment's premises are only partly established, and the reachability of the trailing tool_use state is not decided.
Confirmed: shouldAddUserMessageToHistory skips the user message for empty content on a first attempt, so resumeAfterDelegation → initiateTaskLoop([]) adds no tool-result message. normalizeToolResultPairing leaves a trailing assistant message unchanged, and its own doc comment states this is intentional because the results "have not been produced yet." When the history ends with an unmatched assistant tool_use, the request sends that state unchanged.
Not established: whether the history can end that way at resume. resumeAfterDelegation does not flush pending tool results. It saves the in-memory history and continues. Pending results live in userMessageContent until flushPendingToolResultsToHistory persists them. The reachable case is therefore a delegation flush that fails and is not handled by the caller. The caller of resumeAfterDelegation and its handling of a failed flush were not inspected.
The proposed remedy is also not specified. "Complete the pairing at the send boundary when no tool execution remains pending" gives no way to tell a resumed delegation from a turn whose tools are still pending. That conflicts with the normalizer's documented contract, so a safe edit cannot be drawn from the comment as written.
Decisive missing evidence: the call site(s) of resumeAfterDelegation in the delegation flow, and whether a failed flushPendingToolResultsToHistory result lets the parent resume with unsaved tool results. Until that is read, the finding cannot be posted as a defect.
