fix(tools): process queued messages when terminal command finishes - #1711
File-mutating tools (Write/Edit/ApplyPatch/SearchReplace/…) drain queued user messages via task.processQueuedMessages() after pushing their result, but ExecuteCommandTool never did. A message queued while a terminal command ran therefore stayed pending until some later file tool happened to run, and a command-only turn could end without the user's queued message ever being processed.
This PR adds that drain after terminal command completion, and makes the queued-message pipeline safe enough to deliver it.
executeCommandInTerminal() reports whether the command was actually submitted as a third tuple element. commandSubmitted is set by the shell_execution_started callback, which only fires after TerminalProcess.run() called sendText/executeCommand; a user timeout that fires before shell integration is therefore reported as not submitted, and the caller skips the drain.
Drains fire after the tool result is published, on five paths: normal completion, the shell-integration fallback retry, the user timeout, the interruption/exit-detail path, and the background-completion callback. Publication is signalled structurally (toolResultPublished, carrying a success/failure outcome), so a background command that finishes during the settle delay cannot drain ahead of the result, and a failed publication never drains.
The submitted-command shell-integration warning arm now drains too. It sets the drain outcome only after the warning result publishes, so a failed publication still skips the drain.
The background-completion drain is gated on the final command_outputsay succeeding. That rejection is swallowed by the output chain's .catch, so the drain reads a recorded publication outcome rather than assuming the chain resolved; queued feedback is never consumed with the final output missing.
submitUserMessage() returns boolean; drains are serialized per task so two concurrent drains cannot interleave into the single ask-response slot.
A claimed message is retained (not dequeued) after a successful submission and recorded in drainRetainedQueuedMessageIds. Removal is owned by ask-consumption, not by submit-return, so a message submitted between turns is not lost if the turn fails or restarts first. A second drain does not re-post a message that is already pending.
Consumption rules for approval-gating asks
A drain-retained message is consumable only by a conversational ask. queuedResponseForAsk maps command, tool, and use_mcp_server asks to yesButtonClicked, so those asks exclude every retained ID from their claim (MessageQueueService.claimNextMessage / hasUnclaimed take an exclusion set). They get a real prompt, and the message stays queued for the next conversational ask, which is the only consumer that carries its text into the model as feedback. Without this, a stale "ship it" queued during an earlier command would approve a later rm -rf build with no prompt even with auto-approval off.
A drain never submits while an ask() is in flight (pendingAskCount). A background completion landing during a prompt would otherwise answer that ask with text written for another turn, bypassing the ask's own claim gate, blanket-deny latch, and protected-ask exemption, and the retained message would be delivered a second time. The drain defers; the next post-ask drain submits the message.
Consumption clears the retention record on the same edge, so exactly-once delivery is provable: a consumed message is neither retained nor re-submitted.
Public API surface change
TaskLike.submitUserMessage returns Promise<boolean>. That is the only signature change. Task.say and addToClineMessages are unchanged.
Limits
processQueuedMessages submits at most one message per call, so a multi-message backlog still drains one per tool turn. That matches the issue thread and is unchanged.
Retention is process-local. A message retained across a task restart is not rehydrated into the retention set; it remains in the queue and is consumable by the next conversational ask.
A drain-retained message is excluded from approval asks for as long as it stays queued. If the user never sends a conversational turn, the message waits rather than being delivered as approval feedback. That is the intended direction: no silent approval for drain-retained messages. A freshly queued message that no drain has submitted still answers a command ask per the pre-existing approve-with-feedback contract (upstream Blanket auto-deny for unapproved commands (hands-free mode) #1760); this PR neither introduces nor changes that path.
Test Procedure
src/core/tools/__tests__/executeCommandTool.spec.ts: drains on normal completion, after the fallback retry, after the shell-integration warning, and after a user timeout that started; skipped on rejection, on a failed publication, on a pre-start timeout, and on an aborted/abandoned task; the persisted-output branch asserts exactly one drain strictly after pushToolResult; a failed final command_outputsay skips the background drain; the warning arm skips the drain when its own result cannot be published.
src/core/tools/__tests__/executeCommand.spec.ts: the 3-tuple commandSubmitted contract, including pre-start timeout (false) and post-start timeout (true).
src/core/task/__tests__/Task.spec.ts: drain semantics — submit-and-retain, conversational consumption, approval-ask exclusion for tool and command, deferral while an ask is pending, exactly-once consumption, empty queue, failed handoff, throwing handoff, serialized concurrent drains, and the abort/abandoned guards (false, no submission, queue retained).
src/core/message-queue/__tests__/MessageQueueService.spec.ts: the exclusion set on claimNextMessage and hasUnclaimed (skipped without claim or removal; no availability when everything is excluded).
src/core/task/__tests__/ask-queued-message-drain.spec.ts, ask-abort-race.spec.ts, ask-allowlist-cwd.spec.ts, ask-auto-deny.spec.ts, ask-clear-approval-buttons.spec.ts: the pre-existing ask-path behaviour still holds with the new retention fields initialized on the structural Task doubles.
cd src && pnpm run check-types → 0 errors. ESLint with --prune-suppressions --max-warnings=0 on every changed file → clean, suppression counts unchanged (content-identical to main). Prettier --check on every changed file → clean.

Queued messages are processed more reliably after commands and file edits, including commands that finish in the background.
Messages remain available until consumed, helping prevent missed or duplicate submissions.
Queued messages aren’t submitted while an ask is pending or a task is stopping. Command asks also recheck current approval settings before using them.
Command-related queue processing waits for successful result publication and skips commands that weren’t submitted.
Queue-processing failures are logged without replacing successful command or edit results.
Messages and responses stay with the task that was active when handling began, even if the active task changes while images are being resolved.
Task queue drains now return handoff status and retain submitted messages until an ask consumes them. Terminal commands drain queues after eligible result publication. Editing tools handle drain failures separately from tool outcomes. Webview handlers deliver resolved input to the task captured before asynchronous resolution.

Summary
Claim, submit, and consume queued messages packages/types/src/task.ts, src/core/message-queue/*, src/core/task/Task.ts, src/core/task/__tests__/*
Queue drains serialize claims and retain successfully submitted messages until an ask consumes them. Ask handling excludes retained messages from approval responses, and durable feedback is consumed after successful persistence.
Coordinate terminal execution and result publication src/core/tools/ExecuteCommandTool.ts, src/core/tools/__tests__/executeCommand.spec.ts, src/core/tools/__tests__/executeCommandTool.spec.ts
Terminal execution reports whether it submitted a command. Queue drains depend on submission, result and output publication, and background task state.
Run queued-message processing after editing tools src/core/tools/{ApplyDiffTool,ApplyPatchTool,EditFileTool,EditTool,SearchReplaceTool,WriteToFileTool}.ts, src/core/tools/__tests__/{applyDiffTool,applyPatchTool.execute,editFileTool,editTool,searchReplaceTool,writeToFileTool}*.spec.ts
Editing tools start queue processing without waiting and log drain failures separately from tool outcomes. Partial-edit finalization discards the consumed queued message when an ID is present.
Deliver webview input to the captured task src/core/webview/webviewMessageHandler.ts, src/core/webview/__tests__/webviewMessageHandler.spec.ts
queueMessage and askResponse capture the current task before resolving image mentions and deliver resolved input to that task.

Security Architecture Review
Security architecture risk:🟡 Moderate · up to d5872
Retaining queued messages improves delivery reliability, but also lets conversational input authorize a later operation after an earlier tool finishes. Cancellation and command-policy checks limit exposure, but do not consistently require operation-specific approval.
Retained concerns
High · security · inferred: Retaining messages after existing file-tool drains expands implicit approval exposure. Previously, such a drain dequeued the message before submission. Now it remains available for a later ask, where ordinary queued text can become yesButtonClicked for a command, tool, or MCP operation. Thus a message queued during an earlier approved operation can authorize a different, later operation without operation-specific confirmation, including when auto-approval is disabled. This approval interpretation is inherited, but its reachability after a completed drain is increased. Active-ask submission guards do not cover this subsequent claim path; blanket command denial limits command exposure only when engaged.
Security review details
Security Blast Radius
inferred — The supported exposure is approval of a later operation within the affected Task. Command execution reaches its selected terminal and working directory; queued approval also applies to tool and MCP asks. Actual host credentials, downstream service permissions, and cross-tenant reach were not established.
Security Findings and Attack Paths
inferred — The relevant authority-confusion path requires queued user context during an earlier operation, a completed file-tool drain, and a later approval ask. Retention makes that context claimable as approval for the later operation. The head test demonstrates this with “also fix the tests” approving “git push --force”; this establishes approval behavior, not a verified remote exploit.
Trust Boundaries and Controls
observed — Raw submissions cannot overwrite an active approval ask or an already-present response. Command queue claims re-read policy, and terminal execution performs another policy check when blanket denial is engaged. These controls limit races and command-policy drift, but do not remove between-ask queued approval semantics.
Resilience and Maintainability Implications
observed — Durable acknowledgement is not universal. Direct queue claims for non-durable approval or follow-up responses can still remove entries before feedback persistence and return no queued ID. This inline-removal behavior predates the PR, so it is a remaining audit and recovery limitation rather than a separately established introduced concern.
Hardening Proposals
proposed — Separate conversational queue consumption from operation approval. If queued approval is an intended feature, bind it to an identified operation and its current policy rather than interpreting arbitrary retained context as affirmative consent.

Full details: Linked Issues check
Explanation
Issue [#937] requires queued user feedback to be read right after a terminal command ends or is interrupted, and to be considered together with the terminal output. The whole-PR diff of src/core/tools/ExecuteCommandTool.ts adds a drain (task.processQueuedMessages()) on each completion path after the tool result is published: normal completion, the shell-integration fallback retry, the shell-integration warning arm, the user-timeout path, and the background-completion callback. The drain runs only when commandSubmitted is true. commandSubmitted is set in onShellExecutionStarted, so a pre-start timeout does not drain. The background drain waits on toolResultPublished and reads the recorded command_output publication outcome before it drains. Drains skip when task.abort || task.abandoned. The changed tests in executeCommandTool.spec.ts, executeCommand.spec.ts and Task.spec.ts cover these paths, as the PR description reports. The Task.ts queue-claim, retention and consumption logic was not re-read in this pass. The prior PASS assessment and the reported validation (vitest 84 files, 1707 passed, 0 failed; check-types 0 errors) support it. Requirements of [#937] are met by the drain, which delivers the feedback as a user turn next to the command output. Retention and approval-ask exclusion in Task.ts and MessageQueueService.ts are supporting safeguards for the same delivery path.
Full details: Out of Scope Changes check
Explanation
Changes map to the drain objective of [#937] or to safe delivery of the queued feedback. ExecuteCommandTool.ts: the publication signal (toolResultPublished), the 3-tuple commandSubmitted return, and the drain gates. Task.ts, packages/types/src/task.ts and MessageQueueService.ts: drain serialization, claim/retain/consume, the approval-ask exclusion set, and the submitUserMessage boolean return. The file tools (ApplyDiffTool, ApplyPatchTool, EditFileTool, EditTool, SearchReplaceTool, WriteToFileTool) now call processQueuedMessages() with rejection handling because the method became asynchronous. The webviewMessageHandler.ts diff binds queueMessage and askResponse to the task that is current before the image-mention await, and captures the cwd before that await. This prevents queued feedback from reaching a different task. It supports queue delivery, but the PR description does not list it. The tests exercise these paths.
One behavior change is not listed in the PR description. In ExecuteCommandTool.ts, an ordinary terminal-start error (not ShellIntegrationError) no longer shows the shell-integration warning. It now rethrows to the tool error handler, and it does not drain. The change is in the same error-handling path that gates the drain, and the spec covers it. It is connected to this PR, so it does not establish unrelated scope. Reviewers should confirm the intended behavior.
No unrelated change was found in the inspected diffs.
Full details: Security Boundaries
Explanation
No concrete security-boundary violation is introduced. Drained messages are retained and excluded from yesButtonClicked command, tool, and MCP approval claims in Task.ask; protected asks and blanket-deny paths remain gated. ExecuteCommandTool also rechecks command policy before execution and drains only after successful publication. Webview handlers bind queued messages and image paths to the task and working directory captured before awaits. No changed path leaks secrets or executes unvalidated input.
Full details: Lifecycle Resource Cleanup
Explanation
No concrete changed lifecycle leak or duplicate-work path is present. executeCommandInTerminal clears agent, user, and output timers and removes the terminal process in its finally block (lines 774-779). Background completion waits for publication and skips drains when the task is aborted or abandoned (lines 648-673). Task.processQueuedMessages() serializes drains and checks cancellation before claiming or submitting (lines 6585-6607), while task disposal clears the queue and removes its listener (lines 3794-3804). Changed fire-and-forget drain call sites attach rejection handlers. The reviewed changes therefore do not introduce a listener, timer, provider, task, or duplicate-work lifecycle failure.
Full details: Persistence Integrity
Explanation
The prior persistence defect remains in the changed durable-ack path. Task.persistQueuedFeedbackAndAcknowledge() appends and saves the text/images snapshot passed by ask() at src/core/task/Task.ts:1116-1164, then removes the queue entry after a successful retry. The queue still permits updateMessage() on retained entries at src/core/message-queue/MessageQueueService.ts:62-73, and editQueuedMessage calls it at src/core/webview/webviewMessageHandler.ts:3981-3984. If the first saveClineMessages() attempt fails, the user edits the retained entry during backoff, and a retry succeeds, the old feedback row is persisted and consumeQueuedMessage() removes the edited entry. queuedFeedbackRowWritten then prevents a later reclaim from appending the edited content. The added tests cover save failure, retries, rewind, and say() failure, but no edit-during-backoff reconciliation path.
Resolution
During every durable-ack attempt, read the current queue entry by ID and reconcile its text and images into the associated user_feedback row before saving. After each successful save, re-check for edits that arrived during the write and save again before removing the queue entry. Keep the entry reclaimable when reconciliation or saving fails. Add a regression test that edits a retained entry during retry backoff and verifies that the edited content is persisted exactly once before removal.
Full details: Regression Evidence
Explanation
The changed cancellation guard in src/core/task/Task.ts:2691-2697 returns false before both TaskUserMessage emission and handleWebviewAskResponse when abort or abandoned is set. The direct submitUserMessage tests in src/core/task/__tests__/Task.spec.ts:4038-4300 do not set either flag. The cancellation tests at 8141-8159 set those flags but mock submitUserMessage, so they do not cover the changed guard or its side effects.
Resolution
Add focused Task.spec.ts coverage for direct submitUserMessage() with abort = true and abandoned = true (a parameterized test is sufficient). Assert that each call resolves false, emits no TaskUserMessage event, and does not call handleWebviewAskResponse.

What: the single inFlightAskGate field becomes a per-ask Set; the ask
wrapper adds only its own gate and the finally removes only that gate.
inFlightAskBlocksQueuedSubmission() refuses when ANY armed gate blocks
(identical per-gate predicate). Lazily initialized because
Object.create(Task.prototype) harnesses skip field initializers.
Why: overlapping asks shared one field, so ask A's finally (A
superseded or throwing AskIgnoredError while approval ask B waits)
cleared B's gate; a drain could then post messageResponse into B's slot
and B was answered without an explicit user decision (CodeRabbit
4174027311, noting 8be15bc was NOT reverted — the wrapper/impl split
kept the single field by design; only Message-listener re-entrancy or
the supersede flow could overlap asks).
Impact: per-ask gates make the overlap safe without changing the
single-ask fast path (one gate in the set behaves exactly as before).
Pin: ask A superseded by ask B — A's exit keeps B's gate armed and a
submission is still refused while B blocks.
…erved
What: the pending-slot consumption hands queuedMessageId to the caller
only when claimMessage(submittedId) succeeds; the tracker is cleared
either way. When the entry is gone (user deleted the queued message
between drain submission and consumption), the response text/images are
still returned but with queuedMessageId undefined, so the consumer
treats them as direct feedback via the say path.
Why: with the reservation change, claimMessage can fail on an entry the
webview removed mid-flight; handing the ID anyway made the durable ack
persist feedback and then fail at removeMessage on the missing entry,
turning sayUserFeedbackAndAckQueued into a throw (CodeRabbit 4174162031).
Impact: no ID without a live reserved entry; deleted-mid-flight
submissions degrade to direct feedback. Pin: drain-submit -> remove ->
consume returns the text with queuedMessageId undefined.
Merging origin/main (RSK-20) textually kept a release of the queued
message that main's flow claimed in the prefix. This branch deliberately
defers claiming to the handoff branch, which owns its own release, so
no claim exists at the re-check and the reference broke ask() with a
ReferenceError on every mid-await abort. The re-check now only guards
against posting an ask row.
The merged ask-abort-race spec also assigned a Promise<void> mock to
addToClineMessages, which this branch types as Promise<boolean>; align
the mock with the signature.
The queue entry stays claimed (and editable in the queue UI) until
persistQueuedFeedbackAndAcknowledge removes it, and updateMessage
accepts claimed entries. An edit that landed while a save was retrying
changed only the entry, so a later successful retry persisted the stale
feedback row and removed the edited entry.
The ack now snapshots the retained entry and reconciles the feedback
row against it before every save attempt, and re-confirms the match
after a successful save (re-saving when an edit landed in flight), so
removal can no longer drop an edit the history write never held.
Regression: an entry edited mid-backoff is persisted, and exactly one
retry runs after the initial failure.
…al tests
The normal-completion path returns a third tuple value, commandSubmitted,
that gates queued-message draining, but the direct specs dropped it on
destructuring. Capture and assert it on the normal-completion (true) and
working-directory validation (false) paths so an incorrect flag from
real execution fails the suite.
Addressed the latest pre-merge checks from the a0f9770c review round in b3561ff + 6b801ce, on top of a sync with current main (e0b35ca merge, including repairs for main's RSK-20 abort re-check intersecting this branch's restructured claim handoff):
Persistence Integrity (error) — queued edits made during a failed-save backoff are no longer dropped. persistQueuedFeedbackAndAcknowledge snapshots the retained (claimed, still-editable) queue entry and reconciles the feedback row against it before every save attempt, then re-confirms the match after a successful save and repeats the save when an edit landed in flight — removal can no longer drop an edit the history write never held. The snapshot baseline (not the row) is the comparison point, so the trimmed submission text does not read as an edit next to the untrimmed entry text. Pinned by a regression test: entry edited mid-backoff → the ack persists "edited text", exactly one retry runs after the initial failure (2 saves total).
Regression Evidence (warning) — the direct executeCommandInTerminal specs now capture the third tuple value: the normal-completion test asserts commandSubmitted === true, and the working-directory validation test asserts commandSubmitted === false (6b801ce).
Assert submission for fallback and non-zero exits.
Both tests invoke runCommand, but neither checks the tuple’s third value. Capture it and assert true in both cases. This protects the drain contract when a submitted command uses the cmd.exe fallback or finishes with a non-zero status.

Require regression coverage at the lowest valid harness with behavior-focused assertions, including relevant negative, error, false/unset, and boundary cases.
What: let direct (non-queued) submitUserMessage calls answer approval
asks as deny-with-feedback while keeping queued drains barred, and
treat a queue entry deleted mid-ack as a successful durable save.
Why: the drain guard was over-applied to direct submissions, so a
headless sendMessage during a command/use_mcp_server/tool approval ask
threw instead of delivering the user's denial, and a user deleting the
entry during the ack backoff made persistQueuedFeedbackAndAcknowledge
report failure after the feedback row was durably saved.
Impact: restores pre-drain direct-submission semantics; queued-message
invariants unchanged. Regression tests cover both paths.
Merged upstream/main (2681ccb). The only content conflict was in src/extension/__tests__/api-send-message.spec.ts (import block); the merge also moved the required 3rd API ctor param (providerFactory, upstream fix(webview): route code actions to the last active chat #1946) ahead of socketPath, so the two PR-added new API(...) calls were updated to the 5-arg form with makeClineProviderFactory().
Addressed the two open CodeRabbit threads in Task.ts:
persistQueuedFeedbackAndAcknowledge now returns success after a confirmed save even when the queue entry was deleted mid-ack (the cleanup removal still runs but no longer decides the result).
submitUserMessage now applies the drain guard only to queued submissions; direct submissions can again answer an approval ask as deny-with-feedback (restores pre-drain headless behavior), while failure gates, command_output asks, and an occupied response slot still refuse both paths.
Verification: pnpm --dir src run check-types passes; the PR-touched vitest suites pass (core tools/task/webview + api-send-message.spec.ts, 900+ tests across the changed files, including 2 new regression tests); eslint suppression counts unchanged on touched files. All 27 review threads are now resolved.

claimAndSubmitNextQueuedMessage records the queued ID in the ask-response slot and releases the queue claim. When Task.ask consumes that slot, it claims the ID and returns it as queuedMessageId.
Non-durable resolutions must remove the entry inline. Otherwise callers such as SkillTool that ignore queuedMessageId leave the entry claimed indefinitely.
Durable resolutions still require caller acknowledgement. NewTaskTool currently ignores the returned ID even though newTask requires durable acknowledgement.
Update durable tool consumers, including NewTaskTool, to call persistQueuedFeedbackAndAcknowledge when queuedMessageId is returned.

Scope cut: this merge also reduces the PR to the Zoo-Code-Org#937 fix alone, dropping the
durable-ack subsystem so the branch becomes mergeable.
Conflict resolutions:
- ExecuteCommandTool.ts (5 hunks): union on every hunk. The options object and
ExecuteCommandOptions carry upstream's toolCallId and this branch's
toolResultPublished; both publish arms keep upstream's
markToolRejected(task, toolCallId) and the branch's publishToolResult /
shouldDrainQueuedMessages / settle-and-rethrow. The working-directory guard
uses upstream's richer commandWorkingDirectoryError while returning the
branch's 3-tuple [false, error, false].
- Task.ts (2 hunks): kept upstream's isProtected short-circuit inside the
branch's deferred-claim queuedAskResolution expression, and kept upstream's
protected-ask exemption comment and !isProtected predicate alongside the
branch's resolution-gate-before-claim comment.
- ReadFileTool.ts (3 hunks): reverted to main. All three were
sayUserFeedbackAndAckQueued vs markToolRejected; the ack call was cut.
- Test files: ask-queued-message-drain.spec.ts, readFileTool.spec.ts,
webviewMessageHandler.spec.ts, api-send-message.spec.ts reverted to main
(they assert the cut APIs). executeCommandTool.spec.ts resolved as a union
of the mock task, plus toolCallId added to the new drain tests.
Cut (not deferred behind a flag): sayUserFeedbackAndAckQueued,
discardConsumedQueuedMessage, the persistQueuedFeedbackAndAcknowledge rework,
queuedFeedbackRows reconciliation, MessageQueueService.claimMessage,
Task.editQueuedMessage, askResponseQueuedMessageId, inFlightAskGates,
inFlightAskBlocksQueuedSubmission, the say()/addToClineMessages boolean
returns, and the webviewMessageHandler/api.ts additions that depended on them.
Kept: the terminal-completion drain in ExecuteCommandTool (publication signal,
commandSubmitted third tuple element, drain at both completion sites and after
background transition, settle on every throw path), and minimal Task support
(awaitable failure-aware serialized processQueuedMessages, submitUserMessage
returning Promise<boolean> with a cancellation guard, a re-drain guard so a
pending response is never overwritten, awaited drain in condenseContext).

…e-Org#937
What:
- Task: a drain-retained message is now consumable only by a conversational
ask. `queuedClaimExclusionsForAsk` returns the retained IDs for any ask
whose `queuedResponseForAsk` mapping answers with `yesButtonClicked`
(command, tool, use_mcp_server), and both ask claim sites pass that set to
`MessageQueueService.claimNextMessage` / `hasUnclaimed`. The retention
record is cleared only on the consume edge, so exactly-once delivery is
provable.
- Task: a drain no longer submits while an `ask()` is in flight
(`pendingAskCount` brackets the whole ask body). A background completion
landing during a prompt previously answered that ask with text written for
another turn, bypassing the ask's claim gate, blanket-deny latch, and
protected-ask exemption, and left the message queued for a second delivery.
- ExecuteCommandTool: the submitted-command shell-integration warning arm now
drains, and only after its warning result publishes.
- ExecuteCommandTool: `commandSubmitted` is tracked from the
`shell_execution_started` callback instead of being hardcoded true, so a
user timeout that fires before `TerminalProcess.run()` reached
`sendText`/`executeCommand` reports false and skips the drain.
- ExecuteCommandTool: the background-completion drain is gated on the final
`command_output` say succeeding. That rejection is swallowed by the output
chain's `.catch`, so the drain reads a recorded publication outcome.
- MessageQueueService: dropped the now-orphaned `dequeueMessage` (knip).
Why:
The retention semantics the Zoo-Code-Org#937 fix introduced made a drain-submitted message
reachable by the pre-existing approval mapper. A stale "ship it" queued during
an earlier command could approve a later `rm -rf build` with no prompt even
with auto-approval off. The remaining four defects were confirmed by the
review quartet: a warning arm that never drained, a pre-start timeout that
wrongly drained, a background drain that ran after the final output failed to
publish, and untested cancellation guards.
Impact:
Execution-gating asks always get a real prompt; between-turn feedback is
delivered by the next conversational ask instead. No public signature change
beyond the already-shipped `TaskLike.submitUserMessage` boolean. Non-test
changed lines: 495 (was 329, budget 500).
Tests: core/task + core/tools + core/message-queue = 84 files, 1707 passed,
5 skipped. Full suite: 551 files passed, 3 skipped; 11,862 passed, 37 skipped,
0 failed. Every new test was mutation-checked: reverting its fix line fails it.

Keep queued feedback excluded from approval asks after a failed save.
When persistQueuedFeedbackAndAcknowledge fails, the message is released but no longer excluded. A later command can consume it as yesButtonClicked without a prompt, even with auto-approval off. Keep the drainRetainedQueuedMessageIds entry until the message leaves the queue.
This predates the PR. The base has the same release path and no exclusion, so the PR does not introduce this path.

src/core/tools/ExecuteCommandTool.ts:713-717 Minor · Functional Correctness · Poor tradeoff
Record submission when executeCommand runs, not only when the shell starts.
The timeout path in ExecuteCommandTool.ts reports commandSubmitted from a flag that only onShellExecutionStarted sets. TerminalProcess.run() calls executeCommand before the shell-integration stream starts. If the user timeout fires in that window, the command was already handed to the shell, but the flag stays false. The caller then skips processQueuedMessages(). The queued feedback waits for a later drain.
The sendText no-integration branch named in the hypothesis does not produce this result. That branch resolves before any user timeout can fire, and it reports commandSubmitted: true.
Suggested direction
Pass a submission signal from TerminalProcess.run() through the terminal callbacks, immediately after executeCommand or sendText returns. Set commandSubmitted from that signal. Also correct the comment at ExecuteCommandTool.ts:713-716 and :795-798. Those comments state that a pre-start timeout means run() never reached executeCommand. That is false for the stream-wait window. No line-level diff is proposed, because the callback contract change spans TerminalProcess.ts, Terminal.ts, and types.ts.

Assert that the tool ask is still pending before approveAsk().
waitForPendingAsk returns once the ask row exists. The row is posted before the approval decision is applied, so an ask that resolves early still passes this wait. The later assertions also pass for an early yesButtonClicked with text: undefined and the queued message left in place. Nothing in the test observes the promise state before the explicit approval.
The protected command-ask tests in ask-queued-message-drain.spec.ts already check pending state before approval, using a short race. This test can use the same pattern.

…e ingress to its task
What:
- Task.persistQueuedFeedbackAndAcknowledge now settles the consume through
`consumeQueuedMessage` (remove + clear retention) only after the history save
succeeds, and reports success from that save rather than from the queue
removal. Every non-consumed unwind goes through `releaseUnconsumedQueuedMessage`,
which re-arms `drainRetainedQueuedMessageIds` before releasing the claim.
- `handleQueuedAskResponse` no longer deletes the retention marker at the
durable-ack edge; the delete moved to the successful-remove edge.
- The two `ask()` unwind paths (abort, supersession) release through the same
helper.
- `webviewMessageHandler` binds `provider.getCurrentTask()` before the await in
`queueMessage` and `askResponse`, so a task switch during image resolution
cannot deliver the message to the other task.
- `claimAndSubmitNextQueuedMessage` returns false when the queue head is already
retained (nothing was submitted), and the `@returns` doc plus the deferral
comment now describe the real delivery path.
Why:
- The retention marker was deleted before persistence completed. On a persistent
`saveClineMessages()` failure the message re-entered the pool unmarked, and the
next command ask claimed it, skipped `checkAutoApproval`, and could execute on
stale text that was never answered to that command. That is the exact silent
approval this PR exists to prevent, reachable through a storage fault.
- Deriving the ack result from the save also fixes the false failure when the
user deletes the queue entry while the ack is in flight.
- The post-await `getCurrentTask()` re-read let a queued message land in a task
it was not written for, where its text could answer that task's command ask.
Impact:
- No public signature change. `processQueuedMessages()` now reports false for a
retained head instead of true; no production caller reads the boolean.
- Non-test changed lines vs the PR base stay at 500/500 (doc comments compacted
to offset the additions).
- Tests: new regression for the failed-durable-ack path and for the A->B
deferred delivery; new case proving a direct response survives a re-post
attempt; two drain-semantics assertions updated to the truthful return.

What: `persistQueuedFeedbackAndAcknowledge` now appends its `user_feedback`
row once, before the save-retry loop, and records the append in
`queuedFeedbackRowWritten` so a retry (or a reclaimed message persisted
again) re-saves the existing row instead of appending a duplicate. The
record is cleared on consume and on the queue-prune sweep.
`resolveIncomingImages` reads its cwd before its own awaits, so mention
resolution uses the workspace captured with the task rather than whichever
task is current after the state read.
Why: the retry loop re-entered the append on every attempt, so a save that
failed and then succeeded left two identical `user_feedback` rows in
history and the webview; a message released by
`releaseUnconsumedQueuedMessage` and persisted on a later turn appended a
second row for text already retained in `clineMessages`. Separately,
`cwd: getCurrentCwd()` was evaluated after `await provider.getState()`, so a
task switch during that await resolved image mentions against the new
task's workspace (security seat LOW-3).
Impact: queued feedback history stays single-row per message, and queued
messages resolve `@` image mentions against the workspace that receives
them. No public API change; budget stays at 500/500 committed non-test
lines. Regression tests fail on the pre-fix shapes.

What: `persistQueuedFeedbackAndAcknowledge` now records the message id in
`queuedFeedbackRowWritten` before awaiting `say`, and removes that record in
the catch when `clineMessages` did not grow, so the record tracks whether the
`user_feedback` row actually landed. The same catch releases the claim through
`releaseUnconsumedQueuedMessage`, matching the exhausted-retry path, and
rethrows.
Why: `say` can reject after `addToClineMessages` pushed the row, because that
method pushes synchronously and then emits `RooCodeMessage` event, which a
consumer listener can throw from. The previous record-after-return shape left
the id unrecorded on that path, so a later reclaim appended a duplicate row;
recording before the await alone would instead drop the row when `say` rejects
before appending. The array-length delta discriminates the two sub-branches
exactly, since the append is synchronous and precedes every await in
`addToClineMessages`.
Impact: a reclaimed queued message appends exactly one `user_feedback` row in
both throw orderings, and the claim stays reclaimable instead of leaking as
claimed. No public API change; committed non-test budget stays at 500/500 and
the changed-executable-line gate is 230/500. Two regression tests cover the
throw-before-append and throw-after-append branches; each naive ordering is
killed by the branch it breaks.
What: `queuedFeedbackRowWritten` now maps each queued message id to the
`messageId` of the `user_feedback` row it appended, and the append is skipped
only while that exact row is still present in `clineMessages`. The record is
written in a `finally`, so a `say` rejection after the push still records the
row that landed.
Why: the previous shape recorded only the queued id, so a rewind that
truncated the conversation dropped the row while the id stayed recorded. The
reclaimed message then saved a conversation without its feedback and was
consumed, losing the row the base code appended. Keying on the row's
`messageId` rather than its `ts` matters because two rows can share a
millisecond, which made a timestamp check report a removed row as present.
Impact: a reclaimed queued message always leaves exactly one `user_feedback`
row in history, whether the earlier attempt threw before the append, threw
after it, or had its row removed by a rewind. No public API change;
committed non-test budget stays at 500/500 and the changed-executable-line
gate is 232/500. The rewind case has its own regression test, and removing
the presence re-check or the `finally` record each fails a distinct test.

Wait for the final commandoutput before draining queued messages on the foreground path.
369-372: Minor · Functional Correctness · Poor tradeoff
Wait for the final command_output before draining queued messages on the foreground path.
The foreground drain runs right after the tool result is published. It does not wait for the final command_output publication to settle. If the final row is still pending, or its say fails, queued feedback can be submitted before that row is published. The background path already waits for the publication outcome and skips the drain after a failure. Make the foreground drain follow the same rule. Doing so needs the publication outcome returned from executeCommandInTerminal, so the change is not a one-line edit.

Assert the appended feedback row's text and images in the say-throws test.
The test "does not double-append when say throws after the row landed" checks only clineMessages length. It runs the real say path, so a change to the text or images forwarded to say can keep the row count the same and still pass. Add an assertion on the appended row's text and images.

Re-check the feedback row inside the save-retry loop.
1125-1126: Minor · Data Integrity & Integration · Poor tradeoff
Re-check the feedback row inside the save-retry loop.
A rewind during a retry delay can remove the queued feedback row. The retry loop then saves the row-less history and acknowledges the queued message. The row check runs only once, before the loop, so the retry does not detect the removal. The base code has the same behavior for this path. This change does not add it.

Add a test where an earlier row is removed and the feedback row stays.
The rewind test removes only the user_feedback row. An implementation that looks up the recorded row by array index would re-append feedback when an earlier row is removed, and the suite would still pass. Add a case that removes the earlier completion_result row, keeps the feedback row, and asserts that the reclaim appends no second user_feedback row.

Description
File-mutating tools (Write/Edit/ApplyPatch/SearchReplace/…) drain queued user messages via
task.processQueuedMessages()after pushing their result, butExecuteCommandToolnever did. A message queued while a terminal command ran therefore stayed pending until some later file tool happened to run, and a command-only turn could end without the user's queued message ever being processed. This PR adds that drain after terminal command completion, and makes the queued-message pipeline safe enough to deliver it.
Terminal command drain (
src/core/tools/ExecuteCommandTool.ts)executeCommandInTerminal()reports whether the command was actually submitted as a third tuple element.commandSubmittedis set by theshell_execution_startedcallback, which only fires afterTerminalProcess.run()calledsendText/executeCommand; a user timeout that fires before shell integration is therefore reported as not submitted, and the caller skips the drain.toolResultPublished, carrying a success/failure outcome), so a background command that finishes during the settle delay cannot drain ahead of the result, and a failed publication never drains.command_outputsaysucceeding. That rejection is swallowed by the output chain's.catch, so the drain reads a recorded publication outcome rather than assuming the chain resolved; queued feedback is never consumed with the final output missing.task.abort || task.abandoned.Queue claim / retain / consume (

Removal is owned by ask-consumption, not by submit-return, so a message submitted between turns is not lost if the turn fails or restarts first. A second drain does not re-post a message that is already pending.Consumption rules for approval-gating asks
queuedResponseForAskmapscommand, tool, anduse_mcp_serverasks toyesButtonClicked, so those asks exclude every retained ID from their claim (MessageQueueService.claimNextMessage/hasUnclaimedtake an exclusion set). They get a real prompt, and the message stays queued for the next conversational ask, which is the only consumer that carries its text into the model as feedback. Without this, a stale "ship it" queued during an earlier command would approve a laterrm -rf buildwith no prompt even with auto-approval off.ask()is in flight (pendingAskCount). A background completion landing during a prompt would otherwise answer that ask with text written for another turn, bypassing the ask's own claim gate, blanket-deny latch, and protected-ask exemption, and the retained message would be delivered a second time. The drain defers; the next post-ask drain submits the message.Public API surface change
