feat: add Prometheus metrics endpoint - #224

Closes StreamGive#114 Summary: - expose Prometheus text metrics at GET /metrics without a database dependency - record HTTP request counts and duration by method, route, and status - publish indexer checkpoint, latest-ledger, and lag gauges - count successful and failed webhook notification deliveries - document scraping behavior and add an endpoint smoke test Validation: - npx eslint on all parseable changed files (pass) - npx vitest run test/metrics.test.ts test/notifications.test.ts (3 tests pass) - npm run typecheck (blocked by pre-existing src/indexer/worker.ts syntax error)
