feat(indexer): configurable events-per-poll cap - #220

Add INDEXER_MAX_EVENTS_PER_POLL validation and pass the limit to Stellar RPC. Persist the last handled event cursor so capped polls resume without dropping same-ledger events. Consolidate the duplicate checkpoint model and restore the malformed worker polling loop required for the feature to run.
