checkpointed. No approved algorithm available means the effect is denied, never signed with a refused one and
never written unsigned.

## Project context (Slice 033, as built)

`AGENTS.md` is repository content, not owner-authored configuration. Every file on the path from the session's
project root to its working directory enters the prompt's context tier inside an
`<untrusted source="agents_md" path=... digest=...>` block, capped at 16,384 bytes in total with every cut
stated, and the untrusted rule already in the prompt covers it. The gate never reads the prompt: a file that
says approvals are off changes nothing about what asks (AC3, a test), and the same holds for anything else the
model reads.

## Permission gate (Slice 021)

Every `Trinity.Tools.Tool` declares `effect/0` (`:none | :artifact | :catalog`) and the gate derives risk from the tool **name only** via `Trinity.Permissions.tier/1` (`:read | :write | :exec | :network | :destructive`; unmapped → ask).

`AGENTS.md` (slice 033): the project's instructions to agents, read from the session's
project root down to its working directory and rendered into the prompt's context tier.

**Precedence.** Every `AGENTS.md` on the path from the root to the working directory is
loaded, outermost first; the block says the nearest file wins where they disagree, which
is how the model is told to read them.

**The cap.** The total is capped at `config :trinity, :agents_md, max_bytes:` (16,384,
measured at G1). The nearest file is kept whole first; outer ones are cut from the end, and
every cut states the file and the bytes that went, so the prompt never pretends a file was
complete.

**Provenance.** It is repository content, not owner-authored configuration: every file is
rendered inside an `<untrusted source="agents_md" ...>` block with its digest, and the
prompt's untrusted rule applies. An instruction in it to skip approvals is text the model
reads; the gate never reads the prompt (AC3).

**Live reload.** There is no cache: the Session calls `load/2` when it builds each turn's
request, so a change is in the next turn.
