feat: visualise and calculate the total cost of single tool call / tool defintion over the session #30

For tool definition - track it over all requests, since it would be present from the beginning (for tools/mcps)

For tool call - detect the first request it appears in the context definition, and track till it ends. Multiply token cost by the number of requests it lives in context
