The SWA per-layer path in KVCacheManagerV2 reuses persistent host staging tensors as sources for non-blocking device copies. The overlap scheduler can enqueue the next iteration and rewrite those tensors before the previous H2D copy drains. The resulting page table, copy index, or scratch metadata can describe a later host state, causing attention to read the wrong physical KV pages without an immediate error.
This change snapshots each asynchronous host input into a per-call CPU tensor before enqueueing the device copy:
copy_idx is copied into a fresh host snapshot;
scratch beginnings, ends, and slots are built in fresh host snapshots;
host_kv_cache_block_offsets is copied into a fresh host snapshot.
The device-side SWA conversion and its output contract are unchanged. The persistent host page table remains the source used by the C++ cache manager; only the asynchronous staging inputs are isolated from later scheduler writes.
This is the follow-up called out explicitly by #16088 for the enable_swa_scratch_reuse per-layer path.
Test Coverage
python -m py_compile passes for the modified implementation and regression test.
uvx ruff check passes for both files.
git diff --check passes.
A CUDA stream-stall harness on an RTX 5090 (SM120) reproduces the stock failure: baseline checksum 3048, five raced copies all mismatched with checksums 597333024, 597333792, 597334560, 597335328, and 597336096.
The same harness passes with the snapshot fix: output matches the baseline checksum 3048.
The regression test is included at tests/unittest/_torch/pyexecutor/kv_cache/test_kv_cache_v2_swa_overlap_race.py.
The focused CUDA regression test covers copy_idx, page-table, and scratch-metadata staging. It fails in all 3 cases against stock TensorRT-LLM 1.3.0rc28 and passes in all 3 cases with the snapshot fix on the same RTX 5090 setup. The full repository test session was not run because the installed validation wheel does not provide the repository test conftest extension tensorrt_llm._device_copy.
PR Checklist
PR title follows the required format
PR description explains the failure mechanism and semantic change
The SWA per-layer path now snapshots copy_idx, scratch metadata, and the host page table into fresh CPU tensors before enqueueing asynchronous device copies. Device staging buffers and output processing remain unchanged. The persistent host page table remains available to the C++ cache manager.
The supplied report says the focused CUDA regression and py_compile, Ruff, and git diff --check passed. The full test session was not run because the validation wheel lacked the _device_copy test conftest extension. These results were not independently verified.
QA Engineer Review
The CUDA-only parameterized test, test_swa_staging_snapshot_survives_host_overwrite, queues a copy behind a CUDA event, overwrites one of three host inputs (copy_idx, page table, or scratch metadata), and compares the output with a baseline. It also checks that the baseline contains offsets and validates a scratch offset for the scratch-metadata case. The manager shuts down in finally.
The test is not listed in the inspected integration CI or manual-QA lists. Coverage verdict: sufficient for the three staging-source overwrite cases. The test skips when CUDA is unavailable.
Per-File QA Perspective
tensorrt_llm/_torch/pyexecutor/kv_cache/kv_cache_manager_v2.py — Verify that asynchronous SWA per-layer copies use stable CPU snapshots for the index, scratch metadata, and page table. Device-side conversion and output handling remain unchanged.
tests/unittest/_torch/pyexecutor/kv_cache/test_kv_cache_v2_swa_overlap_race.py — Covers delayed copies followed by overwrites of each staging source. It is not listed in the inspected integration CI or manual-QA lists.

Walkthrough
KVCacheManagerV2 now uses fresh host snapshots for SWA per-layer indices, scratch metadata, and page-table data before nonblocking device copies. A CUDA-gated test overwrites each source during queued copies and compares the results with a baseline.

Summary
Snapshot host inputs before device copies tensorrt_llm/_torch/pyexecutor/kv_cache/kv_cache_manager_v2.py, tests/unittest/_torch/pyexecutor/kv_cache/test_kv_cache_v2_swa_overlap_race.py
Copy-index, scratch-metadata, and page-table transfers use fresh host tensors as sources for nonblocking device copies. A CUDA-gated test overwrites each source during a queued copy and checks the copied offsets against a baseline.
The snapshot implementation is in place, but the race test can miss the regression it is meant to catch. This is a bounded test-confidence risk; merging is reasonable with owner follow-up to make the transfer ordering deterministic.

Issue [#21385] requires stable per-call snapshots for the page table, copy_idx, and scratch metadata in the SWA per-layer path. The implementation creates fresh host snapshots before each asynchronous device copy. The CUDA regression test covers all three inputs and verifies that delayed copies preserve the baseline result after host-source overwrites.

The current implementation builds fresh scratch snapshots from the request descriptors. The test overwrites persistent tensors that this path does not read. For the scratch_metadata case, queue a second copy with a different request ordering before releasing the gate, then check that the first result still matches the baseline. This detects reuse of any shared scratch staging buffer.

Coverage currently includes the copy_idx, page_table, and scratch_metadata parameter cases, but the scratch case does not cover cross-call staging reuse.

Update the `scratch_metadata` case to queue a second copy with reversed request
ordering on `manager._stream` before releasing the gate, using a separate result
buffer and copy index. After synchronizing, verify the second copy completed and
that the first result still matches the baseline, while preserving the existing
checks for other source cases.

Addressed the follow-up. The scratch_metadata case now queues a second copy with reversed request ordering on the same gated stream before releasing the event, checks that the second output is populated and scratch-derived, and still compares the first output with its baseline. Focused fixed validation passes all 3 cases.
