fix(libsy): preserve routing across Codex environment updates - #1191
Codex can send automatic environment updates as user messages, which currently rerun the classifier and reset Stage mid-task. Ignore standalone <environment_context> updates when detecting user turns and selecting the classifier's task messages. Real user follow-ups still trigger routing, and environment context remains in model requests.

Walkthrough
The change updates user-turn detection to ignore tool results and standalone environment-context messages. Task-message selection now uses this detection, and tests cover environment-context messages in task fixtures and profile switching.

is_user_turn excludes tool results and standalone environment-context text. Tests cover standalone context and messages that also contain user content.
task_messages uses is_user_turn while retaining its ordinary-task-content requirement. Fixtures and a profile-switching test cover environment-context messages.
The change stops environment updates from resetting routing, but it also introduces edge cases in the routing and verification paths. A real follow-up that is followed by a context update may not be re-routed, VGR may pick the wrong verification branch, and windowed classifier requests may omit the task. Resolve these before merging.

If the first user message is an environment update, trim_messages anchors on that update. With recent_turn_window: Some(0), the judge receives the update but not the task. Use the same task-selection predicate for the windowed anchor.

Update the task selection used by trim_messages for the windowed anchor to use
the same predicate that identifies the real task, rather than selecting the
first message with Role::User. Ensure recent_turn_window: Some(0) retains the
task when an environment update appears first.
