This PR adds the Settings actions that need input or confirmation: restart the server (y to confirm), change preferences, save, import, or remove a server key, and update from source.

Key actions accept only names that the server config declares with api_key_env, and the form masks the key field. A source update shows a preview and starts only after you type UPDATE. App::start_action matches every ActionKind without a _ arm, and every_controller_action_has_a_settings_key fails when an action has no key. After this PR, the tree matches #975.
