Improve Result Display (UI/UX) and README

Problem:

1. Results are displayed as plain text, making them hard to read.
2. README lacks screenshots, setup instructions, and contribution guidelines.

Proposed Improvements:

Results UI: Use colored cards (✅ True / ❌ False), show confidence score, make responsive, add fade-in animation.
README: Add screenshots, features list, contribution guide, and badges.

Impact

1. Better UX for users checking facts.

2. Easier onboarding for new contributors.
