Add comprehensive design document for enabling fresh Ralph iterations to optionally read historical logs from previous iterations. Key features proposed: - Three modes: off (default), smart (recommended), full - Log indexer to extract structured data from raw logs - LLM-based summarization using local qwen model - Token budget management to avoid context overflow - Relevance scoring to prioritize useful context This addresses the issue where agents get stuck repeating failed approaches because they lack visibility into what was already tried. Inspired by @airesearch12's insight about Claude Code looking back into logs containing complete context of past chat histories.
