• The README index is stale. It lists 419 entries; the directory has 420.
• Two index rows carry the same ID, D-QSP. D-RFT-0 / D-QSP-0: Kleene form of set normalization (SQL NULL), PostgreSQL differential #1668's co-research entry began its title with "D-QSP / D-RFT / D-LFA", so the index derived the same ID as docs(D-QSP/D-RFT/D-LFA): Quack SQL parity, relalg/thought-git, flow plans #1667's plan entry.

Changes:

• Retitled the co-research entry so its title names no D-id. Its index ID now comes from the file name.
• Regenerated .claude/board/entries/README.md. The index reuses each file's existing row, so the stale D-QSP row was removed before regenerating.

Checks, run locally on this branch:

• entries_index.py --check: passes (420 rows).
• supersession_index.py: output is unchanged.

Board hygiene only. No code, plan or contract changes.
