Preserve plan mode state and current plan across /new trajectories #2

Problem

When the user runs /new, the new session has no plan mode state or plan path — both are stored as custom session entries (plot-mode, plot-plan) that don't carry over. Users have to re-enter plan mode and relocate their plan file each time.

Desired behavior

New trajectories should inherit plan mode state and current plan path from the previous session, so planning work can continue seamlessly across conversations.
