PEP 829: Several updates based on DPO feedback (#4898)

* Several updates based on DPO feedback

Specifically, we drop the `site.toml` file proposal in favor of an evolution of the `.pth` file
format and the addition of `.start` files.

Also, update the Discussions-To header and Post-History.
