docs(pm-dispatch): filing-gate first-touch close covers only a card that is none of the four categories

Ruling B on the reach-less finding question: the first-touch close for
missing items applies only to a card outside all four filing-gate
categories. The clause on a finding without reach: leaves the line; such
a finding keeps the triage-duties p3 fold, which this ruling leaves as
written. Line-neutral at the file's ceiling (64/64).
