feat(pm): write-pace — one shared throttle for every seat write
The helper only, ahead of wiring the five write transports: the per-token SHA-256-prefix log, the 3 s gap, the 40/hour budget whose 41st is refused with a prescription, and the stop marker a platform back-off signal writes.
