docs(changeset): the spec headline says which read points moved and which stay

The headline claimed packages/spec/src no longer cites an objectui
File.tsx:NNN for a live read point. Two live citations with no ledger row
stay, as the changeset's own last bullet says, so the headline now states
what the diff delivers: the read points that have a liveness row move into
it, and the two without one stay, named below.
