Subject: [PATCH] ISSUE_TEMPLATE.md: remove retired template

- GitHub stopped reading single-file issue templates on 30 March 2025,
  so this file is never shown. The forms in `.github/ISSUE_TEMPLATE/`
  already cover what it asked for.
